Exertional sickness
Abstract:
The following case serves to remind the physician that persons who are heterozygous for sickle cell disease, a condition known as sickle cell trait, can experience serious and sudden complications. Although sickle cell trait is generally considered benign because reported clinical manifestations are rare, and life expectancy is normal, 1, 2 its complications can be fatal. A 23-year-old African-American man with known sickle cell trait was admitted to our hospital following a syncopal episode. A recruit with the United States Army National Guard, he had collapsed near the completion of a 2-mile obstacle training course while sprinting to the goal line. Although he had quickly awakened, he continued to experience severe pain, weakness, and cramping in his bilateral lower extremities, buttocks, and lower back. He denied chest pain, shortness of breath, or presyncopal warning.
