Abstract:

A 44 year old French Caucasian suffered from acute peritonitis 8 days after a left nephrectomy for renal artery stenosis. Peritonitis was related to a necrotizing enterocolitis. A massive small bowel resection and right hemicolectomy were performed, preserving 85 cm of duodenojejunum. A jejunostomy and transverse colostomy were accomplished. Total parenteral nutrition (TPN) was continued throughout the study (3,000 to 3,500 cal/24 h), the patient body weight remained stable during the study (Tab. 1). Gastric acid secretion, collected through a gastrostomy tube, ranged from 1,500 to 1,800 ml/24 h and had a pH of 1 to 3. Twenty-four hours sustained intravenous cimetidine was started on the 27th postoperative
