Hemodilución normovolémica aguda en cirugías con riesgo de sangrado mayor a 1000 ml de la volemia durante el período perioperatorio: Hospital Vicente Corral Moscoso, Cuenca 2010


Abstract:

Objective. Describe the clinical, haematological and probable adverse effects of patients who undergo acute normovolemic hemodilution in surgical procedures with bleeding of more than 1000 ml of total blood volume during the perioperative period. Methodology. We conducted a descriptive study of 32 patients who underwent acute normovolemic hemodilution, leading to a hematocrit of 30%. The volume replacement was performed with colloid. We evaluated the heart rate, systolic blood pressure, diastolic and mean pressures, electrocardiogram, arterial blood gases and hemoglobin and hematocrit values. Observed adverse effects and also reported the use of allogeneic transfusions. Results. We observed a lack of knowledge and skepticism because this type of autotransfusion. Both blood pressure and heart rate decreased during periods of hemodilution and post hemodilution this being statistically significant (P = 0.0001), but these changes did not pass on the patient clinically. The arterial pH and PaO2 had no changes but there was a decrease in the values of PaCO2, HCO3 and BE, which were statistically significant (P = 0.0001). Decreased hemoglobin and hematocrit values indicating that patients were haemodiluted. No adverse effects were observed either during or after completion of the hemodilution. Was obtained allogeneic blood savings by 78.4%. Discussion. Acute normovolemic hemodilution is a type of autotransfusion that does not produce adverse effects on patients if performed properly and aims at saving allogeneic blood DECs.
