IMPROVING PATIENT HEALTH BEHAVIOR IN PRIMARY CARE THROUGH SELF-REGULATION


Abstract:

Introduction: programs that improve patients' self-regulation skills improve health outcomes. Objective: to compare health behavior self-management action plan initiation rates for primary care patients. Methods: A randomized controlled comparative effectiveness trial was conducted, evaluating the impact of 8-week EMAP versus CBD. Results: EMAP participants reported a higher rate of action plan initiation compared to CBDs (57.6% [n = 53]). Conclusions: EMAP facilitates chronic disease self-management and represents a compelling model for dissemination within primary care patient-centered medical homes.
