Idiopathic hirsutism: Flutamide efects in the treatment
Abstract:
Several studies have reported that flutamide, non steroidal antiandrogen drug, seems to be free of any other hormonal or antihormonal activity. It has been proposed as a useful drug in the therapy of women hirsutism. We studied a total of the women with the clinical and endocrinologic dianostic of idiopathic hirsutism, after exluding other possible causes of hirsutism. All the patients were clinically (Ferriman-Gallwey gradint scale and hair shaft diameter measurement) and endocrinologically evaluated before starting the treatment. They received 500 mg daily of flutamide (250 mg twice daily). The evaluation was repeated at 6 months of therapy. A significant decrease in the Ferriman-Gallwey score was observed flutamide treatment. There were equally a significant reduction in total hair shaft diameter, whereas the cortical diameter didrit support a significant decrease. The administration of flutamide during 6 months did not change the basal LH and FSH concentrations nor the responses of both gonadotropins to GnRH simulation. The plasma androgen values did not change significantly after the flutamide treatment, except for androstenediol glucuronide. We observed a significant decrease of this which seems reflecat an activity of the drug over 5α-reductase level. The hormonal responses after ACTH stimulation, previous dexametasone suppression, were similar before and after the treatment with flutamide, except for dehydroepiandrosterone sulfate response, which was significantly lower after 6 months of therapy with flutamide. Since this, a positive role of this drug in the treatment of skin androgenic alterations of an adrenal source can be deduced.
