Incidencia de abdomen agudo ginecológico en la Maternidad Enrique C. Sotomayor periodo 2014
Abstract:
Gynecology and Obstetrics acute abdomen is a medical emergency that requires immediate therapeutic attention taking into account that not all end in surgery. Its management includes surgical, medical and gynecological conditions, which can endanger the life of the patient and the fetus in the case of pregnant women. According to the National Institute of Statistics and Censuses (INEC), in 2010 in Ecuador 3156 cases of Acute Abdomen, 2374 cases of ruptured ectopic pregnancy (EER) and 79 cases of ruptured ovarian cyst (QOR) were reported. About 13% of women with symptoms of Acute Abdomen are related to gynecological problems, including around 10% of women with a clinical diagnosis of acute appendicitis actually have a gynecological emergency (11), however we do not know the rate of Gynecological Acute Abdomen who come to the Enrique C. Sotomayor Maternity Hospital as an emergency. The symptoms may include sudden, intense colic-type pain, located in the iliac fossa or hypogastrium, but it is generally unilateral, as well as it may present nausea, vomiting and general condition compromise. A correct study of the patient should be carried out, starting with a good medical history, physical examination and when suspected of Broken Ectopic Pregnancy, other diagnostic tests should be included, including beta Human Chorionic Gonadotropin (GCH), transvaginal ultrasound and diagnostic laparoscopy. If not diagnosed early, it can lead to hemorrhagic shock. Likewise, the ovarian cyst, its rupture generates a lot of pain, associated or sometimes confused with Broken Ectopic Pregnancy. Management may include hydration, antibiotic therapy, blood transfusions, and if necessary; surgical treatment. Some cases can be resolved by laparoscopy, while others with laparotomy. It is the leading cause of maternal morbidity and mortality in the United States (9%). Morbidity and mortality depends on the source of infection, previous state of the patient, as well as the first measures used to eradicate the bacteria, all this would help them assess the prognosis of the same.
