Intubation after noninvasive ventilation failure in chronic obstructive pulmonary disease: is it properly evaluated?


Abstract:

Emergency department (ED) is the more crucial hospital step to apply noninvasive mechanical ventilation (NIV) in different forms of acute respiratory failure [1]. We know that some factors and practices may influence NIV failure and invasive mechanical ventilation, but there is scarce information on whether the factors could be useful for proper decision-making in high-risk patients in ED [1]. We have read with great interest the study by van Gemert et al.[2] that explores original determinants of NIV failure. Although this study led to considerable controversy on the methods for early diagnosis and preventive approaches, we believe that some aspects need to be taken into account for a proper clinical practice extrapolation.
