Left Atrial Appendage Closure: Technical Considerations of Endocardial Closure
Abstract:
Atrial fibrillation is the most common clinically encountered arrhythmia. 1 The primary goal of therapy is to reduce rates of stroke and thromboembolism, and atrial fibrillation is ideally treated with oral anticoagulant therapy (OAC). 2 However, long-term compliance remains a significant issue and some patients are not candidates for anticoagulation. Left atrial appendage closure (LAAC) has been shown to be an effective method for reduction of cardioembolic stroke risk in patients who are not candidates for long-term anticoagulation. 3–5 An endocardial LAAC implant, although intuitive, requires a sound understanding of left atrial and left atrial appendage (LAA) anatomy. Multimodality imaging can facilitate implant success and efficiency while reducing the risk of complications.
