Obstetric ultrasound reported monochorionic twin pregnancy diamniotic with findings related to syndrome-fetal fetus stage IV transfusion according to Quinteros classification were carried out as a therapeutic measure amniodrenajes serial, which allowed prolong gestation for fetal lung maturity, but not they solved the TTTS. Also we do a review as to the different treatment options today that range from watchful waiting, with a mortality approaching 100% to etiological treatments based on selective laser ablation of the communicating vessels with a survival of the twin least 70%, with a rate of neurological handicap less than 5%.
