Tratamiento de las fracturas de humero distal con doble placa LCP dorsolateral y medial versus una placa dorsolateral Hospital Luis Vernaza, período 2018-2021
Abstract:
Introduction: Distal humerus fractures are complex, open reduction with internal fixation is the preferred treatment. Numerous surgical approaches and fixation techniques have been described with weak evidence of advantages and benefits. Objective: To compare the results of treatment with a double dorsolateral and medial plate versus a dorsolateral plate in trauma patients at Hospital Luis Vernaza, period 2018-2021. Material and methods: A retrospective case series study was carried out, which included patients treated for diaphyseal fractures of the distal third of the humerus. Patients who have had at least a six-month followup. The type of fracture, the length of the plate, the number of fixed screws, and the characteristics of the fracture union were analyzed. In addition, data on range of motion, Disabilities Arm, Shoulder, and Hand Score (DASH), and complications during follow-up were collected to obtain clinical outcomes. Results: A retrospective case series study was carried out, the type of fracture, the length of the plate, the number of screws and the characteristics of the fracture fixation were analyzed. Data were collected on range of motion, hand, shoulder and arm disability score (DASH), and complications during followup. Results: 73 patients were studied, with an average age of 49.15 ± 19.96, the majority male, 67.1%. Traffic accidents are the main cause 35.6%, type A3 injuries 24.7%, A1 20.5% and B3 15.1% predominate. The DASH score was lower in the double plaque group 1.98 vs single plaque 5.9, p value = 0.012. Complications were 28.8%, observing radial nerve injury 6.8%, loss of fixation 15.1%, infections 12.3%, plate failure or rupture 4.1%, post-traumatic osteoarthritis 5.5%, heterotopic ossification 4.1% and reduced range of movements 24.7% . Conclusions: there are statistically significant differences in the DASH score in favor of the use of the double dorsolateral LCP plate and fewer complications. Key words: Distal humerus.
