Ulna bone defect and septic pseudoarthrosis of the radius after Monteggia type IV fracture


Abstract:

Monteggia type IV fracture consists of fractures of the ulnar and radial shafts with an anterior radial head dislocation, an infrequent entity associated with numerous nervous, vascular, and infectious complications. There are still many challenges around1-4. We present the case of a 37-year-old patient with a bone defect of the ulna and septic pseudoarthrosis of the radius as complications of a Monteggia type IV fracture, treated with free vascularized fibular graft for both injuries, with satisfactory results at 10-year follow-up. The infrequency of this case, whose diagnostic and therapeutic difficulties represent a challenge for trauma specialists, as well as its management and resolution with the vascularized fibula, a technique that has gained notoriety in the reconstruction of long bones, highlight the relevance of this work.
