“Heart team” a new concept of management of complex cardiovascular diseases: A purpose of a case


Abstract:

The multidisciplinary approach in the decision making of complex cardiopathies results in a new scenario inside contemporary cardiology. The “Heart Team” emerges as a multidisciplinary team of clinical cardiologists, echocardiographers, cardiac surgeons and hemodynamists that combine their clinical experience with scientific evidence, clinical conditions and the value of the patient in light of the effectiveness of their care. In heart failure, the concept of “Heart Team” began with heart transplantation programs, where the clinical decision involved the best therapies and the forms of accompaniment for severe patients. A case of a 59-year-old patient with complex valvular pathology that debuted with complicated decompensated heart failure with dilated cardiomyopathy, arrhythmias, left ventricular dysfunction and pulmonary hypertension. The patient was under chronic drug treatment with diuretics, beta-blockers and oral anticoagulation was surgically resolved with native valve replacement. The local team followed the initial opinion of a clinical cardiologist, supported by a third-level institution where a cardiac surgery team was available. While highlighting the importance of an early clinical and complementary diagnosis and the multidisciplinary approach required by this group of patients. Due to the lack of availability at the Minister of Public Health of Ecuador to make the global trend of “Heart Team” a reality. It continued to be a challenge to solve such complex heart diseases to improve the survival of the population.
