“Pancreatitis aguda en el embarazo”
Abstract:
Acute pancreatitis is a rare complication of pregnancy which usually occurs in the third trimester of pregnancy, 68% of cases due to lithiasis. The incidence of acute pancreatitis (AP) in pregnancy is approximately 0.03 to 0.09%. Objective: To describe a clinical case of acute pancreatitis in pregnancy. Material and methods: retrospective descriptive study, presentation of a clinical case. Results: We describe the case of a 31-year-old woman, 35 weeks of gestation, who presents abdominal pain in the epigastrium accompanied by nausea that repeatedly arrives at vomiting, leukocytosis with deviation to the left, simple CT image studies and Contrast of abdomen concludes acute type D pancreatitis due to areas of necrosis of more than 50% of the surface of the pancreas with high tomographic severity index, Balthazar criterion 9/10, which establishes the diagnosis and decides to admit it hospital to complement exams and perform proper management of the patient and interruption of pregnancy due to fetal compromise. Conclusions: The differential diagnosis should be established with acute cholecystitis, acute appendicitis, mesenteric infarction, complicated ectopic pregnancy, hyperemesis gravidarum, preeclampsia and acute fatty liver, among other pathologies. The diagnosis can be confirmed with the serum measurement of amylase or lipase, which will be elevated. The treatment is similar to that of non-pregnant patients and fundamentally of support. In addition, a study of the fetal condition must be performed through ultrasound and fetal monitoring.
