“Shock hipovolémico por adenocarcinoma patrón endometroide infiltrante”


Abstract:

The following clinical case of a female patient 34 years of age is presented. She came with to the emergency room with abdominal pain and genital bleeding with an evolution period of 12 hours. She was admitted to the internal medicine department for genital bleeding instead of the gynecology services department due to lack of space. An obstetric ultrasound is performed and it reports uterine myomatosis, in the right ovary teratoma. A pregancy test is negative. The physical examination showed she was hypotensive, tachycardic with generalized paleness, sore, dry oral mucus. An emergency uterine cavity examination was performed that identified a vaginal mass. Patient persists with bleeding, so colloids, crystalloids, plus two concentrates of red blood cells were administered to temporarily compensate for shock. The patient is sent to an internal medicine ward, where she remains with a prescription of vital signs control with continuous monitoring. Bleeding is being controlled as well as anything injested, and the amount of urine being discharged. After this she is transferred to the gynecological service department where she shows signs of hypovolemic shock. It is decided that a total hysterectomy plus bilateral ooferectomy and a 1/3 exeresis of vagina must be perfomed. Concluding with post-surgical diagnosis of vaginal cancer plus hypovolemic shock. Patient that comes with histopathological result that reports infiltrating endometroide Adenocarcinoma, being referred as SOLCA. The presentation of this case aims to analyze the quality of care, to correlate the clinic with complementary examinations necessary for the identification of a hypovolemic shock and the application of an adequate treatment that does not allow complications to be reached in the patient that may harm her life.
