Cervical funneling in the pbkp_rediction of imminent preterm delivery in symptomatic patients


Abstract:

Objective: To determine the prognostic utility of cervical funneling in the pbkp_rediction of imminent preterm delivery in symptomatic patients. Methods: Women with single-fetus pregnancies, of 24-35 weeks were selected, with threatened preterm delivery and intact membranes. Before the start of any treatment, all were submitted to a transvaginal ultrasound examination to determine the presence of cervical funneling. The main variable of the study was the frequency of imminent delivery (in the 7 days following evaluation). Results: 481 women were selected, 119 participants presented imminent preterm delivery (group A) and 362 patients presented deliveries beyond 7 days (group B). The interval between evaluation and delivery was 4.3 +/- 1.6 days in group A and 56.3 +/- 27.2 days in group B (p <0.0001). Frequency of cervical funneling was also higher in patients in group A (61 cases, 53.9%) compared with patients in group B (18.7%, p <0.0001). Patients with a positive sign presented significant differences in the time interval between evaluation and delivery, genital bleeding, weight of newborns at birth, cervical length and delivery in the following 7 days (p <0.0001). The value under the curve for pbkp_rediction of imminent preterm delivery was 0.679 (95% confidence interval, 0.619 - 0.739). Conclusion: Ultrasound determination of cervical funneling is not useful in the pbkp_rediction of imminent preterm delivery in symptomatic patients.
